GCF Grievance Form

  This is a sample of the information needed should you feel a formal grievance should be filed with the GCF. Provide the information required by this form and any other information you feel is
necessary to clearly relay the full nature of the issue. Submit this completed information to any GCF Officer. That Officer will alert the other Officers that a grievance has been filed. The Officers will investigate the issue and will, 1) Take whatever corrective action deemed necessary, and 2) Advise all concerned persons, in writing, the findings of the investigation and the corrective action, if any, being taken.
